Verification for Residency Onboarding Process

Congratulations on your Match!  Please forward all enrollment and graduation verification requests for your program’s residency onboarding process to the Registrar.  Include your program’s contact information and instructions for submission.

The Registrar’s Office will complete the following requests BEFORE graduation:

  • Letters of Good Standing / Enrollment Verification on medical school letterhead

  • Forms requiring anticipated date of graduation, school seal, etc.

Official Transcripts:

Official transcript requests are handled by the University of Hawaiʻi at Mānoa Registrar’s Office.  Click here to order your transcript.

Final transcripts with your “degree awarded” printed on the transcript will be released approximately 6-8 weeks after graduation.

Please submit your verification requests early.  All verifications will be sent out AFTER graduation in the order they are received.


  • Official Transcripts:

  • Graduation Verification Letter:
    Verifies dates of enrollment, graduation date, and degree earned.  Click here to request a letter.

  • Diplomas:
    All diplomas are handled by the University of Hawaiʻi at Mānoa Registrar’s Office.  Diplomas will be mailed to the mailing address on file approximately 10 weeks after the end of the semester and are mailed via USPS.

    *If proof of your degree, prior to the release of your diploma is needed, request for a Graduation Verification Letter from the JABSOM Registrar’s Office.

  • Certified Diploma Copy:
    Please email a scanned copy of your diploma to the JABSOM Registrar (craigtm@hawaii.edu).  Once received, we will certify it and send it directly to the requestor.

When you are ready to apply for your Postgraduate Training License (PTL), please submit the following to the JABSOM Registrar’s Office:

  1. Form MED
    • Complete the top portion of the form ONLY

    • Email the form to Craig Morimoto at craigtm@hawaii.edu

  2. Certified Diploma Copy
    • Email a copy of your JABSOM diploma to Craig Morimoto at craigtm@hawaii.edu

    • If you have not received your diploma, request for a Graduation Verification Letter.  The letter will be sent with your Form MED.  Once you receive your diploma, email a copy to craigtm@hawaii.edu and we will resubmit the Form MED with your certified diploma copy.

  3. Official Transcripts

All completed documents will be uploaded to your application via the Medical Board of California – Direct Online Certification Submission (DOCS) system.
